Accepted Insurance Plans
If you have any questions about your insurance and potential coverage, please call our authorization coordinator at
(928) 771.7577, or fax us at (928) 441-1522. You may also email us at
pmiauth@cableone.net. We can contact your insurance company to determine eligibility of your plan. We will need the name of your carrier and your policy identification number.

If your insurance company is not listed, payment is due upon testing. We will be more than happy to file your insurance paperwork directly for any potential reimbursement by your insurance company. If you do not have insurance, we accept cash, credit and debit cards. Discounts may apply for cash payment.
